FIRST TIME USERS: Participation in your employer sponsored reimbursement plan(s) allows you online access to INNOVA’s password protected web site, where you can keep up with various benefit news, review your year-to-date claims and reimbursements, and access to plan information and forms. To register for online access follow the steps below:
- Click on REGISTER in the gray login box above.
- Step 1: Enter your Login ID, SSN (no dashes or spaces) and click
- Step 2: Enter your Employer Code (if you do not have a code contact an INNOVA representative)
- Step 3: Set up you new Login ID
- Create your Personal Login ID following questions 1-3 and click SUBMIT.
- Step 4: Set up your Password
- Create your Personal Password following questions 1-2 and click SUBMIT PASSWORD.
- Welcome, John!
- Confirm your role: Employee for ABC Company
- (Administrator: INNOVA Benefit Services) and click SELECT ROLE.
You are now logged into INNOVA Benefit Services Information Center. You have access to the site and will not have to repeat the first time login in again. If you have any questions, please call e-mail us at firstname.lastname@example.org or call us at 866-993-7702.
By logging into your account you will have access to your personal account and be able to complete a personalized, online claim form.
If we have your email address, after you fax your claim form you will be automatically notified when we receive your claim form and receipts. General forms are also available, without logging in.
